Overview
We are looking for a Pharmacist to join our team of providing the highest quality of care, reliability, and support to our patients, caregivers, and families. In this role, the Pharmacist will be responsible for dispensing medications, providing clinical advice, as well as delivery services.
Schedule:
Sunday - Wednesday (On-call)
6:00pm - 10:00am (just need to come in per on call order)
Saturday/ Sunday (1 - 2 weekends per month)
12:00pm - 4:00pm rotation
(8-16 hrs per month)
(nightly on-calls 7 days a week)

Responsibilities
  • Provides clinical consultations to patients as well as recommendations to nurses, physicians, and other hospice personnel
  • Evaluates medication profiles, compliance, and modifications to achieve desired therapeutic outcomes
  • Answers customer questions and resolves customer issues in a timely manner
  • Retrieves data in the pharmacy system
  • Manages medication orders
  • Ensures proper storage and security conditions for medications
  • Maintains a productive, orderly, and error-free work environment

Qualifications
  • Doctor of Pharmacy Degree or Bachelor of Science Pharmacy Degree
  • Current and good standing Pharmacist License
  • Strong communication skills, customer service, and problem-solving skills
