About This Access Center Registered Nurse (RN) Position

HopeHealth is seeking an experienced Registered Nurse (RN) for its referral center based in Providence, RI.

The Access Center RN position acts as a customer service liaison between referral sources, care providers, professional staff, patients and families to ensure the smooth and timely admission of patients onto our hospice service.

This is a full time 40 hours per week position, the start time for this position varies, 8-4:30, 10-6:30 rotation every third week) with a every 3rd weekend rotation.


What will you do as a Access Center Registered Nurse (RN)?

  • Demonstrates commitment to patient care utilizing outstanding nursing assessment and communication skills throughout the entire referral and admissions process.
  • Schedules visits for referred patients within required timeframe. Provides follow up on pending patients and non-admissions. Communicates prospective patient updates to appropriate referral sources.
  • Helps resolve referral and admissions scheduling issues and customer concerns through direct communication with the patients, referral sources, and Manager of Access.
  • Provides clinical follow up with referral sources on upcoming discharges from hospitals.
  • Admissions duties as needed
