Job description

At Brighton/Suncrest Hospice, our goal is to change the expectation of hospice care in your area by providing exceptional care and service to our patients.  This is achieved by allocating the resources to increase our staff to patient ratios, thereby increasing clinical visits while lowering clinician caseloads.   • Lead CNA position that requires previous CNA leadership and/or hospice experience
  • Receive callouts from CNA field staff and reassign patient visits as needed
  • Ensure that the daily schedule of patients is complete and updated in database. Assist with staffing assignments for new admissions and scheduling as needed/directed by supervisor.
  • Respond to phone calls from all disciplines, family members and referral sources to assist with concerns or general CNA related questions. Follow up with staff to provide support and education as needed.
  • Assist with the preparation and facilitation of new-hire orientations for all CNA field staff
  • Complete weekly reports which will be addressed in a weekly meeting with direct supervisor
  • Assist in reviewing and exporting all visits in our database, communicating with CNAs in reference to their documentation and timecards and then reporting this information to supervisor weekly.

**Must have previous leadership/management and/or hospice experience*

  • Ability to work well within a team environment but also able to work independently
  • Excellent communication skills
  • Prior hospice experience is preferred, but not required
  • Strong familiarity with computer applications and spreadsheets
