Job Title: Physical Therapist (PT) – Home Health Care

Role Overview

Are you a Physical Therapist who thrives on one-on-one patient care and the freedom of the open road? We are seeking a compassionate and autonomous Physical Therapist to join our home health team. In this role, you won't just be treating symptoms; you'll be helping patients regain their independence in the place they feel most comfortable—their own home.

What You’ll Do

Restore Independence: Conduct comprehensive initial evaluations and develop personalized plans of care focused on functional mobility and safety.

Clinical Excellence: Provide high-quality manual therapy, therapeutic exercise, and gait training tailored to the home environment.

Collaborative Care: Coordinate with physicians, occupational therapists, and nursing staff to ensure a holistic approach to patient recovery.

Documentation: Maintain timely and accurate clinical records (OASIS experience is a major plus!) via our electronic medical record system.

Education: Empower patients and their families by teaching home exercise programs and fall prevention strategies.

What We’re Looking For

Credentials: Valid State License in Physical Therapy (PT).

Experience: Home Health experience preferred, but we are willing to train the right "hospital-to-home" transitioner.

Independence: A self-starter who can manage their own schedule and travel to various patient homes.

Communication: Strong interpersonal skills to build rapport with patients and families.

Reliability: Valid driver’s license, reliable transportation, and auto insurance.
