Summary

GENERAL SUMMARY:

Provides medical services to assigned patients under the supervision of a licensed physician within the supervising physician’s scope of practice. Responsible for providing acute, chronic and preventative health care in collaboration with the cardiologist in both outpatient and inpatient settings. Manages patients by assessing, diagnosing, coordinating care, educating, and documenting pertinent information with the supervising physician to meet patient care, clinical, hospital and national guidelines. Serves as a resource and educator for patients, family members, and fellow staff.

Services include but are not limited to clinical evaluation of patients; ordering, conducting and interpreting diagnostic and laboratory tests; prescription of pharmacologic agents and nonpharmacologic therapies; and teaching and counseling individuals and groups.

PRINCIPAL JOB FUNCTIONS:
  1. 1. *Commits to the KRMC mission, vision, values and goals and consistently demonstrates our core values.
  2. 2. *Performs initial and ongoing physical assessments, identifies potential health-related problems and health risk profiles.
  3. 3. *Works autonomously and in collaboration with the supervising physician to assess and diagnose patients and formulate treatment plans that include pharmacological, diagnostic, and therapeutic procedures and treatments.
  4. 4. *Works autonomously and in collaboration with the supervising physician to assess and diagnose patients and formulate treatment plans as below:
    1. a. Diagnoses patients by synthesizing and analyzing the collected data; formulates a differential diagnosis based on the history, physical exam and diagnostic test results.
    2. b. Identifies and implements a patient treatment plan which may include the ordering of diagnostic and therapeutic procedures and treatments; selecting appropriate pharmacologic and nonpharmacologic interventions; developing a patient education plan; and making appropriate treatment referrals.
    3. c. Implements patient care plans based on current care guidelines and clinical specialty expertise; accurately conducts and interprets diagnostic tests; accurately prescribes pharmacologic and nonpharmacologic therapies; provides relevant patient education and makes referrals to other health care professionals and community agencies as needed.
    4. d. Evaluates patient responses to the disease process and therapeutic interventions, interprets diagnostic studies within the scope of practice and communicates progress to supervising cardiologist.
    5. e. Determines the effectiveness of patient treatment plans; documents patient care outcomes; reassesses and modifies treatment plans as necessary to achieve health and medical goals.
  5. 5. *Responsible for evaluating and stabilizing the acutely ill patient including all modalities of ACLS.
  6. 6. *Responsible for admitting and discharging patients, making daily rounds and providing consultative cardiology services for inpatients.
  7. 7. *Responsible for managing outpatient clinic patients, both new and established.
  8. 8. *Responsible for the sharing of information with the interdisciplinary team regarding the patient’s progress and care.
  9. 9. Diagnoses, treats and manages cardiology-related health problems and chronic conditions according to the scope of practice.
  10. 10. Provides short and long-term follow-up care, assesses risk factor modification and tolerance of treatment, and manages health care needs of patients.
  11. 11. *Responsible for accurate and timely documentation in the medical record of all clinical encounters compliant to billing standards.
  12. 12. Fosters the development and educational processes for students and colleagues and serves as a role model for other staff.
  13. 13. Maintains current and progressive standards through participation in continuing educational programs and review of the current literature in accordance with state licensure and facility requirements.
  14. 14. Supervises and interprets tilt table tests and exercise stress testing.
  15. 15. Supervises exercise as well as pharmacologic stress testing with and without myocardial perfusion imaging and echocardiography imaging.
  16. 16. *Interprets/makes clinical decisions regarding cardiac device transmissions including scheduled remote checks, in-clinic device checks, alert transmissions and physiological monitoring transmissions of cardiac devices such as loop recorders, pacemakers and defibrillators. Evaluates for any abnormalities and coordinates a plan of action if required as delegated by and under the guidance of the supervising cardiologist.
  17. 17. *Responsible for managing the practices’ resulting process of any ordered tests/diagnostics with an appropriate plan of care.
  18. 18. *Responsible for managing anticoagulation (PT/INR) encounters for the practice.
  19. 19. Maintains strict confidentiality of patients and families.
  20. 20. Understands the scope of practice and personal/professional limitations and recognizes when to consult with the supervising cardiologist regarding various aspects of patient care.
  21. 21. Covers weekday and weekend on-call coverage with their supervising physician as scheduled. This includes phone calls from the answering service of PVMC, the admission hotline, and staff from all hospital departments. Includes ensuring coverage of all patient visits on the weekend for active inpatients and any required stress test coverage.
  22. 22. *Responsible for inpatient and outpatient office visits and consultations, assisting in surgical procedures, and providing post-operative care for the entire pre and post-operative time period.
  23. 23. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
EDUCATION AND EXPERIENCE:

Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act required. Master of Science degree in Nursing with current Advance Practice Registered Nurse licensure from the State of Nebraska required. Certification with the U.S. Drug Enforcement Administration for prescriptive authority required.

OR

Graduate degree from a Physician’s Assistant program required. Current licensure as a Physician Assistant required. Completion of Bryan credentialing and re-credentialing process required for employment. Certification with the U.S. Drug Enforcement Administration for prescriptive authority required.

OTHER CREDENTIALS / CERTIFICATIONS:

Basic Life Support (CPR) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.

Advanced Life Support (ACLS) is required. Current NIHSS certification is required.
