Job description

Physical Therapist - Home Health

This is a 13-week contract position.

5x8 Days, 8:30 AM - 5:00 PM, weekdays only (no weekends or call).

The Physical Therapist in this position will provide home health care services to patients, which includes assessments, documentation, and development of patient care plans tailored to long-term goals and immediate needs.

Responsibilities
  • Perform assessments and evaluations on patients in a home health setting
  • Develop and implement individualized treatment plans
  • Document patient progress and adjust care plans as needed
  • Provide education to patients and their families on home exercise programs
  • Utilize assistive devices and adaptive equipment effectively
  • Collaborate with interdisciplinary teams for comprehensive patient care
Required Experience / Certifications / Licensure
  • Minimum of 1 year of home health experience
  • Valid West Virginia State License or Compact License (pending license acceptable)
  • BLS Certification (via American Heart Association)
