Now Hiring: Rehabilitation Assistant (Casual)
Location: Point Grey Private Hospital – Vancouver, BC
About Point Grey Private Hospital
Point Grey Private Hospital is a modern, purpose-built long-term care home offering 153 licensed residential care beds, primarily in private rooms.
Renovated and expanded in 2020, this four-story home features large windows, ocean and mountain views, and a comfortable, homelike atmosphere with dining and recreation areas and spacious outdoor patios on each floor.
What You’ll Do
· Deliver rehabilitative therapeutic interventions in accordance with individualized care plans.
· Facilitate group exercise programs that promote mobility, strength, and overall wellbeing.
· Support the development and ongoing modification of resident care plans in collaboration with the interdisciplinary team.
· Design and implement programs to assist clients with complex disabilities in achieving daily living goals and maximizing independence.
· Observe, document, and report changes in residents’ conditions in a timely and accurate manner.
· Maintain and monitor the safety, cleanliness, and inventory of rehabilitation equipment.
· Assist with the coordination of resident admissions, discharges, and transfers to ensure smooth transitions.
· Contribute to comprehensive resident care planning and goal setting.
· Accurately complete electronic documentation using PointClickCare.
· Delegate appropriate rehabilitation tasks to unregulated care providers and provide guidance as needed.
· Prepare reports and contribute to interdisciplinary meetings, including care conferences.
· Promote and support the overall physical, emotional, and social wellbeing of residents.
· Maintain and oversee the walking program under the direction of the Physiotherapist.
· Implement daily group exercise programs developed in collaboration with the Physiotherapist and Occupational Therapist.
· Plan and deliver small group activities in partnership with the Recreation team.
· Track and maintain accurate attendance records for all programs and activities.
· Perform other related duties as assigned.
What You Bring
· Diploma in Rehabilitation (2-year program) from a recognized institution, or equivalent education and/or a minimum of two (2) years of experience as a Rehabilitation/Therapy Assistant.
· Demonstrated ability to uphold and respect the dignity, individuality, and rights of residents.
· Strong commitment to excellence in the care of older adults within a Long-Term Care environment.
· Experience working with residents living with dementia, with a solid understanding of person-centered care approaches.
· Excellent communication and interpersonal skills, with the ability to build and maintain positive relationships with management, staff, residents, families, and external partners.
· Ability to work independently while also supporting, guiding, and collaborating effectively with team members.
· Strong organizational and time management skills, with the ability to prioritize tasks in a dynamic environment.
· Commitment to ongoing professional growth and continuous learning.
· Physically and mentally capable of performing the duties of the position.
· Demonstrates professionalism, empathy, and a compassionate approach in all interactions.
· Acts as a positive role model, promoting a respectful and supportive team environment.
