Job Description:
We are seeking an experienced Acute Care Case Manager to support care coordination, discharge planning, and transition of patients across an acute care hospital setting. The ideal candidate will have strong hospital-based case management experience and be comfortable managing a high-volume caseload.

  • Start Date: ASAP
  • Facility Type: Acute Care Hospital
  • Schedule: Days
  • Weekend Requirement: Every other weekend, based on department needs
  • Floating: No
  • Contract Experience: Seasoned travelers required; first-time travelers not accepted
  • Experience: Minimum 2 years of acute care case management/discharge planning
