Position Responsibility: Responsible for providing the program overview to interested consumers and to assist members who have obtained prior authorization for Personal Care Attendant (PCA) services and their surrogates, administrative proxies, if necessary, in developing the skills and resources to maximize the member's management of personal health care, personal care services, activities of daily living, and activities related to the fiscal intermediary.
Essential Functions: (Core duties or tasks that are fundamental and not marginal to the performance of the job)
  • Perform intake and orientation on new PCA referrals to determine the initial eligibility for services, instruct the Consumer in the rules, policies and regulations, onboard consumer with the Electronic Visit Verification (EVV) system for timesheets, and to describe the role of the person care agency and the fiscal intermediary.
  • Conduct field visits to assess prospective consumer's physical, social, emotional, and environmental status to determine their eligibility and needs in their homes in accordance with contractual requirements.
  • Manage an on-going case load of PCA consumers.
  • Provide functional skills training to instruct the Consumer/Surrogate in the basic requisites of an effective program of personal care services including utilizing the Electronic Visit Verification (EVV) system for completing timesheets
  • Instruct the Consumer/Surrogate on completing time sheets through EVV, understanding the medical issues and limitations of the Consumer and reviewing the responsibilities of the Consumer/Surrogate/Administrative Proxies.
  • Instruct Consumer/Surrogate/Administrative Proxies on how to hire a new Personal Care worker, review paperwork for new hire and provide a new employee packet.
  • Perform quarterly comprehensive skills training on new Consumer/Surrogate/Administrative Proxies and additional training as needed.
  • Perform annual comprehensive skills training for consumers who are with the program for more than (1) year and as additional training is needed.
  • Provide ongoing support to consumer and/or Surrogate in utilizing the Electronic Visit Verification (EVV) system.
  • Offer instruction on issues regarding benefits, durable medical equipment and referrals to other programs.
  • Keep current personal data, progress notes and tracking software information in PCA Program software and case file.
  • Work collaboratively within an interdisciplinary team of Nurses, Skills Trainers and Case Aides.
  • Work collaboratively within the team when difficult scenarios and issues arise within a teammate's caseload.
  • Assist participants with Medicaid applications, securing Social Security and S.S.I., and other related benefits.
  • Other related duties as assigned.

Qualifications: Associate degree and 4 years relevant work required. High School diploma with significant relevant work experience with elders or the disabled may be substituted for degree. Excellent written, verbal, interpersonal and computer skills necessary. Bilingual ability in either Khmer or Spanish is required. Must have valid driver's license and reliable transportation.
Hours: 37.5 hours a week
