Job Title: Licensed Practical Nurse (LPN) - Long-Term Care (LTC)
Location: Manitowoc, Wisconsin
Setting: Skilled Nursing Facility


Schedule
  • Approximately 48 hours per week
  • Variable Evening & Night Shifts
  • 8-hour shifts

Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality care in a Skilled Nursing Facility setting. The LPN will be responsible for delivering direct patient care, administering medications, and supporting the healthcare team in maintaining a safe and comfortable environment for residents.


Key Responsibilities
  • Provide direct nursing care, including medication administration and treatments
  • Monitor and assess residents' conditions; report changes to RN or physician
  • Supervise and assist CNAs with daily care activities
  • Perform clinical procedures such as wound care, catheterization, and specimen collection
  • Maintain accurate and timely patient documentation
  • Ensure compliance with infection control and safety protocols
  • Respond promptly to resident needs and emergency situations
  • Support residents and families through education and compassionate care

Required Qualifications
  • Active Wisconsin LPN license (required)
  • Current BLS/CPR certification (preferred)
  • Experience in Skilled Nursing or Long-Term Care preferred
  • Ability to work evening and night shifts
  • Strong clinical, communication, and teamwork skills
