Assess the patient's clinical needs upon admission and weekly; set goals; recommend appropriate nursing actions to meet those goals. Keep other licensed clinical professionals informed on items related to the patient's care. Monitor and track changes in the client/patient's condition and needs, and keeps the Director of Nursing and any other appropriate personnel informed.Requirements:Current unrestricted Texas Registered Nurse (RN) license, requiredCurrent CPR/BLS Certification (in-person only), requiredNegative TB skin test or chest x-ray within past year, requiredMinimum one (1) year of documented experience in a home health agency, primary care clinic or health facility, preferredCurrent and valid Texas Driver's License, requiredCurrent and valid automobile insurance, requiredMust be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client.
