Licensed Practical Nurse (LPN)

Join Huntsville Health and Rehabilitation, a long-term care and rehabilitation facility dedicated to helping residents recover, regain independence, and improve their quality of life through compassionate, person-centered care.

This role supports both long-term care residents and rehabilitation patients while helping contribute to a positive and team-focused clinical environment.

The Licensed Practical Nurse (LPN) provides direct nursing care to residents while collaborating with Registered Nurses, physicians, and interdisciplinary team members to ensure quality resident outcomes and compassionate care delivery.

Responsibilities include administering medications and treatments as prescribed by physicians, monitoring and documenting residents' health status, reporting changes in resident condition to the supervising RN or physician, assisting with activities of daily living (ADLs) as needed, collaborating with the rehabilitation team to support recovery plans, maintaining accurate and timely documentation in accordance with facility policies and regulations, providing emotional support and education to residents and families, maintaining a safe and clean environment while following infection control protocols, and supporting teamwork and positive communication throughout the nursing department.

Qualifications include a current Licensed Practical Nurse (LPN) license in good standing, strong clinical skills including medication administration and wound care, excellent communication and interpersonal skills, ability to work effectively within a team environment, compassionate, patient-centered approach to care, and BLS/CPR certification required.
