Salary Range: $56,935 – $85,253 annually
 

Position Summary

Tuba City Regional Health Care Corporation is seeking a compassionate and experienced Licensed Master Social Worker (LMSW) – Case Manager to join its Case Management team. This role is responsible for coordinating comprehensive, patient-centered care for high-risk and underserved populations in an outpatient healthcare setting.

The LMSW Case Manager will focus on psychosocial assessments, care coordination, discharge planning, utilization review, and connecting patients with appropriate community and healthcare resources. The ideal candidate will collaborate closely with multidisciplinary teams to improve patient outcomes, reduce barriers to care, and promote continuity of care and patient self-sufficiency.

Key Responsibilities
  • Conduct comprehensive psychosocial and patient needs assessments
  • Develop individualized care plans based on patient goals and clinical needs
  • Coordinate referrals, discharge planning, and community support services
  • Assist patients and families in navigating healthcare and social service systems
  • Monitor patient progress and update care plans as necessary
  • Perform utilization review and assess medical necessity for referrals and services
  • Collaborate with physicians, nurses, behavioral health professionals, and external agencies
  • Facilitate communication between patients, caregivers, and healthcare providers
  • Coordinate outpatient referrals, including Purchased Referred Care (PRC)
  • Support transitions of care and ensure continuity following hospital discharge
  • Maintain accurate and timely documentation in electronic health record (EHR) systems
  • Advocate for patients and promote patient-centered, evidence-based care practices
  • Participate in quality improvement initiatives, reporting activities, and program development
  • Provide emotional support, crisis intervention, and resource counseling when needed
Required QualificationsEducation
  • Master’s Degree in Social Work (MSW) from an accredited institution
Licensure & Certification
  • Current and unrestricted LMSW or LCSW license in any U.S. State or Territory
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA)
Experience
  • Minimum of 3 years of clinical social work experience
  • Experience in case management, discharge planning, care coordination, or utilization review
  • Familiarity with electronic health record (EHR) systems

  • Experience working with underserved or rural populations
  • Familiarity with Patient-Centered Medical Home (PCMH) model
  • Bilingual skills in Navajo, Hopi, or San Juan Southern Paiute preferred
