JOB TITLE: First Steps Service Coordinator
DEPARTMENT: First Steps / Children's Service
SUPERVISOR'S TITLE: First Steps Manager

JOB STATUS: Full-time, Non-Exempt
SCHEDULE: Flexible, based on family needs; travel throughout assigned counties and occasional evening hours required
Summary:
Provide service coordination for infants and toddlers enrolled in First Steps by guiding families through intake, eligibility, IFSP development, referrals, transitions, and ongoing coordination of early intervention services. This role supports families by connecting them with appropriate services, maintaining required records, and ensuring compliance with state and federal First Steps requirements.
Key Responsibilities:
Receive and respond to referrals within 2 business days
Complete intake procedures, obtain informed consent, and explain due process and procedural safeguards
Collect income and health insurance documentation for cost participation
Coordinate assessments, evaluations, eligibility determinations, and IFSP development within required timelines
Convene and participate in IFSP, annual review, and transition meetings
Coordinate service delivery and connect families with financial support resources such as Medicaid and CSHCS
Maintain accurate electronic records in EI Hub and complete required documentation and monthly reports
Support transition planning to Part B, preschool, or other community services before age three
Assist families with exit summaries and access to community resources
Build and maintain professional relationships with families, providers, agencies, and multidisciplinary teams
Provide services in homes and community settings, including evening appointments when needed
Qualifications & Skills:
Strong written and verbal communication skills in English
Working knowledge of First Steps due process, eligibility criteria, and procedural safeguards
Understanding of state and federal regulations related to early intervention records, IFSPs, transition, and confidentiality
Ability to collect, record, and enter required data accurately
Strong computer skills, including word processing, databases, spreadsheets, and tablet/iPad use
Ability to work independently, manage multiple responsibilities, and maintain confidentiality
Physical & Work Requirements:
Valid driver's license and reliable, safe transportation
Proof of vehicle liability insurance meeting Indiana state law requirements
Ability to travel throughout assigned counties and provide coverage as needed
May require overnight travel for out-of-town training
Maintain a clean, safe work environment
Certifications & Training Requirements:
Successful completion of Service Coordination training
Obtain Service Coordinator credential within 1 year
Maintain Service Coordination credential and complete required ongoing training
Comply with all state and local First Steps policies and procedures
Maintain consumer confidentiality at all times
