Direct Client Care:

Provide comprehensive primary care within RN scope (expanded role).

Coordinate care across the Health Centre and external providers.

Provide telephone triage, advice, and follow-up on results and prevention needs.

Deliver client education and health promotion during assessments and groups.

Skills and Qualifications:

RN in good standing with the College of Nurses of Ontario (CNO)

Bachelor of Science in Nursing (BScN)

>1 year in primary care, including:
oAssessment and triage
oChronic disease management (e.g., diabetes, hypertension, asthma)
oMedication/treatment administration (per medical directives)
oSymptom monitoring and clinician updates
oWomenโ€™s health screening and contraception counseling
oTravel health advice and pre-travel immunizations
oPediatric and/or perinatal experience (asset)

Strong assessment, health promotion/education, counseling, and community developmentskills
โ€ข
Ability to work effectively on an interdisciplinary team
โ€ข
Ability to work flexible hours, including evenings and weekends.
โ€ข
Additional languages, preferably Spanish or Arabic, and EMR/Practice Solutions experienceare assets.
