Territory: Cincinnati, Ohio-Downtown, Queens City, Mason, Mainville, W.Chester, Terrace park, Blanchester, Morrow, East End, Mt. Washington


Schedule: 3 days per week
Hours: 2:00 PM - 9:00 PM

How You'll Work

You'll provide skilled nursing care in patients' homes as directed by the attending physician.

You will work autonomously, so you should be comfortable managing your time efficiently and working independently.

You'll be responsible for all practices and duties within the scope of practice outlined by the state.

  • Develop individualized care plans

  • Administer medications as prescribed by the physician
  • Help decrease re-hospitalizations
  • Prioritize visits for high-risk patients
Continuity of Care
  • Manage multidisciplinary care as applicable

  • Coordinate appropriate admissions, transfers, and discharges
Family Educator & Advocate
  • Educate patients and families about disease processes
  • Use teach-back methods to ensure patient and family understanding

Policies & Documentation
  • Complete clinical documentation according to agency protocols
  • Follow Medicare and federal guidelines
  • Collaborate with the interdisciplinary team and physicians to ensure optimal patient care
