Licensed Practical Nurse (LPN) - Home Care
Job Responsibilities:
• Provide direct skilled bedside nursing care and work with patients and families to provide comfort and preserve dignity in the face of terminal illness in the home setting.
• Evaluate patient /family needs in the preparation and executions of the interdisciplinary plan of care for each patient on the team and the effectiveness of treatments and monitors patient's reactions.
• Participates in Nurse LPN on- call rotation/schedule in order to meet the needs of patients and families.
• Administer skilled nursing care with appropriate documentation, under the supervision of a registered nurse and the physician directed plan of treatment.
• Provide nursing services in accordance with physicians' orders, under the direction of an RN, in line with state licensure status, and in compliance with agency policies and procedures, applicable health care standards, and governmental laws and regulations.
• Assess individual status and care/instruction needed by the client.
Qualifications:
• Active License as a Licensed Practical Nurse in the State of Florida.
• Active CPR license.
• High School Diploma or GED.
• At least one year of nursing experience.
• Experience in a Home Health environment.
• Must have reliable transportation, current driver's license, and proof of current auto insurance.
• Excellent organizational and communication skills.
