Pay Range: $27.00-$32.70/hour (based on experience)
Additional Pay: $2.00 Shift Differential
Sign-On Bonus: $15,000 (paid out in equal monthly installments)

Job Summary
The Licensed Practical Nurse (LPN) provides direct nursing care to residents while collaborating with Registered Nurses, physicians, and interdisciplinary team members to ensure quality resident outcomes and compassionate care delivery.
This role supports both long-term care residents and rehabilitation patients while helping contribute to a positive and team-focused clinical environment.
Responsibilities
• Administer medications and treatments as prescribed by physicians
• Monitor and document residents' health status, including vital signs
• Report changes in resident condition to the supervising RN or physician
• Assist with activities of daily living (ADLs) as needed
• Collaborate with the rehabilitation team to support recovery plans
• Maintain accurate and timely documentation in accordance with facility policies and regulations
• Provide emotional support and education to residents and families
• Maintain a safe and clean environment while following infection control protocols
• Support teamwork and positive communication throughout the nursing department
Qualifications
• Current Licensed Practical Nurse (LPN) license in good standing
• Strong clinical skills including medication administration and wound care
• Excellent communication and interpersonal skills
• Ability to work effectively within a team environment
• Compassionate, patient-centered approach to care
• BLS/CPR certification required
