Job Summary

We are seeking a Per Diem Nurse Practitioner or Physician Assistant to join our Hospitalist team at Cooley Dickinson Hospital. This is a swing shift admitting role primarily, from 2:00pm to 12:00am. The NP/PA will work with the Swing and Nocturnist physicians and admit patients from the Emergency Department to the Inpatient Medical Floors. There is a minimum requirement of two shifts per month. We are located on the CDH Main Campus at 30 Locust St, Northampton, MA 01060.
Our Hospital Medicine Nurse Practitioners (NPs) and Physician Assistants (PAs) work collaboratively with physicians, consultants, nurses, case managers, and rehabilitation teams to deliver high-quality, efficient care to hospitalized patients. In this role, the NP/PA will provide cross-coverage for hospitalized patients throughout the shift, and, at times, serve as a first responder for rapid response assessments. This is an excellent opportunity to practice at the top of your license in a collegial, supportive environment with a high degree of autonomy.
Key Responsibilities:
*Perform initial patient evaluations, including history, medication reconciliation, physical exam, diagnostic review, and documentation of H&P with admission orders
*Monitor patient progress and coordinate care plans with attending physicians and interdisciplinary teams
*Document ongoing care through progress notes
*Perform procedures in accordance with clinical privileges
*Manage discharges, including medication reconciliation and coordination of follow-up care to support safe transitions
*Educate patients and families on diagnoses, medications, procedures, and post-discharge care
*Triage and facilitate patient transfers from the CDH Emergency Department, outside hospitals, and outpatient settings
*Communicate with accepting providers to ensure seamless transitions of care
*Participate in Goals of Care / Hospice care discussions with patients and families.
Summary
The Physician Assistant (PA) is a licensed provider. The PA is responsible for the assessment and management of various populations of patients, medical and/or surgical, including diagnostic and therapeutic interventions, development of appropriate plans of care and ongoing evaluation.

-Provides direct care, counseling, and teaching to a designated patient population in the ambulatory, inpatient, operative, and/or procedural setting.
-Performs complete histories and physical examinations.
-Orders, interprets, and evaluates appropriate laboratory and diagnostic tests.
-Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
-Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.
-Consistently provides high quality and timely documentation including admission and progress notes, procedure notes, operative notes and discharge summaries.
-Performs bedside procedures as are appropriate to the patient population.
