Job Summary: Case manages students at assigned private day schools throughout Maricopa County. The district representative will support IEP team process and decisions for students whose significant and profound disabilities require a placement on the continuum of services beyond the public school setting. This person will collaborate with private day school personnel to provide necessary supports to enable students to reach their IEP goals.
Essential Functions: Essential functions, as defined under the Americans with Disabilities Act, may include, but are not limited to, the following tasks, duties, knowledge, skills and other characteristics. This list is ILLUSTRATIVE ONLY and is not a comprehensive listing of all functions and tasks performed by incumbents in this class.
ESSENTIAL DUTIES AND RESPONSIBILITIES generally include the following. Other duties may be assigned:
  • This position reports to Director and Assistant Director about student progress in private placements
  • Participates in all IEP meetings as district representative by contributing behavior and practical expertise and IEP compliance direction
  • Completes portions of IEP on district paperwork
  • Provides case management services for students with a disability including all requirements related to IEP development, provision, and compliance to state and federal regulations
  • Communicates with transportation and completes transportation forms
  • Communicates and completes paperwork from IEPs for Direct Service Claims
  • Facilitates moves from public setting to private day schools for students that require a more restrictive environment by working with parents to tour potential programs and completing IEP paperwork
  • Facilitates moves from private day schools back to the public setting based on IEP team decisions for the most appropriate environment
  • Manage enrollment and withdrawals of students in the district student management system
  • Turns in the original paperwork and information to the district office following district required timelines
  • Maintains accurate census of all students on caseload per district and state guidelines
  • Maintains accurate and complete records required by law, district policy, and administrative regulation
  • Attends district meetings and serves on staff committees as required
  • Strives to build strong, positive working relationships with community providers
  • Maintains all aspect and provision of confidentiality related to (Family Educational Rights and Privacy Act) FERPA
  • Performs other duties as assigned or required
  • Willing to work flexible hours when necessary

KNOWLEDGE AND SKILLS:
  • Knowledge of applicable Federal, state, county and city statutes, rules, policies and procedures
  • Knowledge of applicable Peoria Unified School District and departmental policies and procedures
  • Knowledge of IEP Compliance
  • Knowledge of all areas of disabilities in regards to goal writing, assistive technology and supplemental aids and services.
  • Knowledge of criteria for ESY eligibility.
  • Knowledge of facilitating of successful meetings to accomplish all IEP responsibilities.
  • Skill in establishing and maintaining effective working relations with co-workers, vendors, students, parents, students Private Day School and their staff, educational advocates and attorneys and the general public and others having business with the school district
  • Skill in operating a personal computer utilizing a variety of software applications

MINIMUM QUALIFICATIONS: Certified Special Education Teacher with Cross Categorical K-12, Severe/Profound, or equivalent certificate issued by the Arizona State Department of Education and a minimum of 5 years in teaching special education
OTHER REQUIREMENTS:
  • Must hold or obtain an IVP fingerprint clearance card.
  • Must be able to pass a background check.
  • Must be able to obtain and maintain an Arizona driver's license.
  • May be required to lift and carry heavy, bulky supplies and materials weighing up to 20 pounds.
  • May be required to lift students weighing up to 60 pounds.
  • May be required to work outside normal working hours.
  • May be required to travel to perform work functions
