Job description

Experienced Home Health Patient Scheduler
At Choice Health at Home, exceptional patient care starts with exceptional coordination. We're looking for an experienced, detail-oriented, organized, and compassionate Patient Services Coordinator (PSC) to help ensure our patients receive timely, seamless home health services.
This position requires previous Home Health experience and proficiency with HomeCare HomeBase (HCHB). If you thrive in a fast-paced environment, enjoy solving problems, and love being the connection between patients and caregivers, we'd love to have you on our team.
What You'll Do
As a Patient Services Coordinator, you'll play a vital role in keeping our clinical team organized and our patients cared for by:
  • Coordinating and scheduling home health visits for clinicians
  • Prioritizing new patient admissions to ensure timely starts of care
  • Communicating with nurses, therapists, patients, physicians, and referral partners
  • Managing scheduling workflows within HomeCare HomeBase (HCHB)
  • Providing outstanding customer service to patients and healthcare professionals
  • Assisting with medical records functions through cross-training as a Medical Records Specialist
  • Supporting quality improvement initiatives and agency operations
  • Helping ensure every patient receives the highest level of coordinated care
Clinical (LVN/LPN) candidates may also have the opportunity to perform patient visits as assigned.
What We're Looking For
  • High school diploma or equivalent required
  • Previous Home Health experience is required
  • HomeCare HomeBase (HCHB) experience is required
  • LVN/LPN license preferred, but not required
  • Strong organizational, multitasking, and communication skills
  • Ability to thrive in a fast-paced healthcare environment
  • Reliable transportation and valid driver's license
  • CPR certification required only if performing patient visits
