Job Summary

Evaluates and assesses patients to establish functional,measurable goals to promoterestorationoffunctionalabilities.Provides treatmentaccordingto a written plan of care to meet established goals. Plans for dischargeand communicatesfindings and progress accordingto professional licensure/certification,scope of practiceand departmental/disciplinepolicies and procedures. Produces other reports as needed. May supervise clinical students and support staff including certified/licensedassistants, technicians,and volunteers.Adheres to the ASHA Code of Ethics. Provides a caring relationshipthat facilitateshealth, and healing. Performs other duties as assigned.

Job Responsibilities

  • Evaluates and assesses the patient based on subjectiveand objectiveinformation.
  • Communicatesresults of the evaluation to patient and other caregivers.
  • Provides treatmentaccordingto establishedgoals on plan of care.
  • Documents interventions/carein the patient'smedical record.
  • Evaluates effectiveness of plan of care and reassesses patient'sfunctionalstatus as needed.
  • Provides discharge planning informationwhich includes patient/familyeducation,follow-up, and referral to communityresources.
  • Contributesto the developmentof staff and students.
  • Participates in activitiesdesigned to improve health care delivery. Completes assigned goals.

Experience

Minimum required - Entry Level

Minimum Required - Education - Completion or an accredited Masters or Doctorate degree in Speech language Pathology, Communicative Disorders or Speech and Hearing Science.

Special Skills

Minimum Required - Basic computerliteracy skillssuch as keyboarding,using amouse, and navigating sites.

Licensure Description

SuccessfulcompletionofNational Examinationin SpeechLanguage Pathology.Registeredas a clinical fellow with stateboard of communciation disorders and sciences.
