Job description

About the role
The primary focus of this role is to review and ensure that clients receive the correct medications and dosages in alignment with physicians' orders. This includes making thoughtful recommendations to reduce prescriptions when necessary in collaboration with psychiatrists and medical teams. Additionally, the nurse will provide medication training to staff as needed, as well as guidance on other medical concerns of our clients.

What you'll do
  • Conduct comprehensive monthly medication audits across residential homes to ensure compliance with physician orders, medication administration protocols, California regulations, Title 17 and Title 22 requirements, and organizational policies.
  • Review Medication Administration Records (MARs), physician orders, medication logs, and related documentation to verify accuracy, completeness, and regulatory compliance.
  • Identify medication trends, documentation discrepancies, administration errors, and opportunities for process improvement, implementing corrective actions as appropriate.
  • Collaborate with physicians, pharmacists, administrators, and residential nursing staff to resolve medication-related concerns and promote safe medication practices.
  • Track medication-related compliance metrics, audit findings, corrective action plans, and quality improvement initiatives, providing regular reports to leadership.
  • Develop, implement, and monitor corrective action plans to address audit findings and ensure sustained compliance.
  • Provide clinical coaching, mentoring, and ongoing support to Licensed Vocational Nurses (LVNs), Direct Support Professionals (DSPs), and residential staff to strengthen medication administration practices and clinical documentation.
  • Plan and facilitate quarterly medication management and medication administration training for residential staff, as well as individualized coaching and refresher training based on audit results, regulatory updates, or identified performance needs.
  • Serve as a clinical resource for medication administration questions, best practices, and regulatory compliance.
  • Support continuous quality improvement initiatives by identifying systemic issues, recommending process enhancements, and promoting best practices in medication management across all residential programs.
  • Maintain accurate, organized, and confidential audit documentation and prepare clear, actionable reports with findings, trends, recommendations, and progress updates for executive leadership.
  • Review medication administration records for accuracy and compliance with physicians' orders

Qualifications
  • Associate's or Bachelor's degree in Nursing or related field preferred or equivalent years of work experience.
  • Current and unencumbered license to practice as an RN in the state of California.
  • 3+ year(s) experience clinical or home health work experience as an RN
  • Reliable transportation to visit multiple locations across The Los Angeles area, Lancaster, and Yucca Valley
