Registered Nurse (RN) – Rural Health Clinic

Department: Rural Health Clinic
Status: Full-Time | 1.0 FTE (40 hours/week)
Schedule: Four 10-hour shifts per week (4x10s), weekdays only
FLSA Status: Non-Exempt
Reports To: Manager – Clinic
Location: Pagosa Springs Medical Center – Pagosa Springs, Colorado

Position Summary

Pagosa Springs Medical Center (PSMC) is seeking a compassionate and skilled Registered Nurse (RN) to join our Rural Health Clinic team.

The Clinic RN collaborates closely with physicians, nurse practitioners, physician assistants, medical assistants, LPNs, patient service representatives, and other support staff to coordinate and deliver exceptional patient care. This role serves as a key member of the clinic care team, providing clinical assessments, patient education, triage services, care coordination, medication management, and support for quality initiatives while helping lead day-to-day clinical workflows.

The ideal candidate is an experienced nurse who thrives in a fast-paced outpatient environment, demonstrates strong clinical judgment, and is passionate about providing patient-centered care close to home.


Schedule Highlights
  • Full-time position (40 hours/week)

Key ResponsibilitiesDirect Patient Care
  • Perform comprehensive nursing assessments and reassessments
  • Obtain and document vital signs, patient histories, medications, allergies, and other clinical information
  • Monitor patients for changes in condition and communicate findings to providers
  • Administer medications, injections, immunizations, and treatments as ordered
  • Perform nursing procedures within RN scope of practice, including:
    • Dressing changes
    • EKGs
    • Nebulizer treatments
    • Ear lavage
    • Suture and staple removal
    • Orthopedic device fitting
    • Other clinic-based nursing procedures
Patient Triage & Care Coordination
  • Triage patient concerns utilizing approved protocols and clinical judgment
  • Respond to patient phone calls, portal messages, and clinical inquiries
  • Coordinate referrals, follow-up care, testing, and treatment plans
  • Assist patients in navigating their healthcare needs and treatment plans
  • Support continuity of care through collaboration with specialists, hospitals, and community resources
Patient Education
  • Educate patients and families regarding:
    • Medical conditions
    • Medications
    • Treatment plans
    • Preventive care
    • Diagnostic testing
    • Wellness strategies
  • Reinforce provider instructions and support patient understanding of care recommendations
Team-Based Care
  • Collaborate closely with providers, medical assistants, LPNs, and support staff
  • Assist in prioritizing daily clinical activities and patient needs
  • Serve as a nursing resource and mentor for clinical support staff
  • Participate in onboarding and training of new team members
  • Promote a culture of teamwork, communication, and service excellence
Quality & Compliance
  • Support quality improvement initiatives and regulatory readiness
  • Maintain accurate documentation in the electronic health record
  • Follow National Patient Safety Goals and infection prevention standards
  • Perform CLIA-waived testing as appropriate
  • Support preventive care and population health initiatives
Education
  • Graduate of an accredited Registered Nurse (RN) program required
  • BSN preferred but not required

Experience
  • Minimum two (2) years of nursing experience in a clinical setting preferred
  • Outpatient clinic, family practice, primary care, or physician office experience preferred
  • New graduates may be considered
  • Experience with:
    • CLIA-waived laboratory testing
    • Immunizations and Colorado Immunization Information System (CIIS)
      preferred but not required

Licensure & Certifications
  • Current Colorado Registered Nurse (RN) license in good standing required
  • Current AHA BLS certification for Healthcare Providers required

Compensation & Benefits
  • Base Compensation Range: $31.96 - $49.41 hourly, based on 0-22 years of relevant experience. "Relevant experience" means relevant to the employee's responsibilities set forth in the job description as determined by the HR Manager, CAO, applicable department manager/director and applicable senior leader. 
  • Benefits: Benefits include generous paid time off, separate sick leave, health, dental, vision, life and AD&D insurance, long-term disability and option for short-term disability, and retirement plan with employer contribution.
