Registered Nurse (RN)

Position Summary:
A well-established Skilled Healthcare Facility located in Staten Island, NY is seeking a compassionate and detail-focused Registered Nurse (RN) to join its interdisciplinary care team. This position centers on conducting comprehensive clinical assessments and developing individualized care plans using the New York State Uniform Assessment System (UAS-NY).

The selected candidate will play a key role in evaluating participants, coordinating care, and ensuring compliance with state and program standards while promoting overall participant well-being.

Key Responsibilities:
  • Conduct comprehensive, face-to-face clinical assessments for current participants in accordance with UAS-NY guidelines, agency protocols, and New York State regulations.

  • Complete UAS-NY evaluations to determine eligibility and continued participation in the program.

  • Assess prospective participants by reviewing clinical, psychosocial, and environmental factors to determine appropriate levels of care.

  • Collect and interpret clinical data from various sources to support sound, evidence-based care decisions.

  • Develop and update individualized service plans tailored to participant needs.

  • Communicate effectively with participants, families, healthcare providers, and interdisciplinary team members to ensure accurate and coordinated care.

  • Demonstrate understanding of the complex needs of individuals with chronic medical and bio-psychosocial conditions.

  • Maintain compliance with all regulatory, documentation, and organizational standards.

Qualifications:
  • Active New York State Registered Nurse (RN) license required.

  • Current UAS-NY Certification required.

  • Experience working with UAS-NY strongly preferred.
