Job Summary

MDS / MDS COORDINATOR

Position Purpose

Conducts and coordinates the completion of required Resident Assessment Instrument (RAI) and is responsible for the implementation and ongoing evaluation of each resident's comprehensive plan of care.

RequiredQualifications

  • Mustpossess a current, unencumbered, active license to practice as a nurse in thisstate. Registered Nurse Licensed preferred.
  • 3years' experience as a nurse, RN preferred, one of which includes supervisoryexperience. Experience with MDScompletion preferred.
  • ResidentAssessment Coordinator (RAC) training must be completed within six months ofhire. RAC Certification is preferred.

Major Dutiesand Responsibilities

Coordination of the facility's Resident AssessmentInstrument (RAI) process in accordance with state and federal regulations.

Accurate completion of all MDS assessments and anysupporting assessments or clinical documentation.

Implementationand ongoing evaluation of each resident's comprehensive plan of care.

Scheduling and leading interdisciplinary care plan teammeetings.

Auditing medical records for the presence ofsupporting documentation for all items coded on the MDS. Provide education to department heads,physicians, and other staff as needed.

Collaborating with health plans, physicians and theinterdisciplinary team for assuring insurance approvals and/or that residentsmeet eligibility and coverage criteria.

Interpreting rules, regulations and coverageguidelines and acting as primary resource for problem solving regarding the SNFprospective payment system and quality reporting program.

RAI/MDSCoordinator Assigned Tasks

    * Utilizes the current RAI Manual as a resource during theassessment coding process.

    * Schedules the Assessment Reference Dates (ARDs) forpayment assessments in a manner that accurately captures each           resident'sclinical characteristics for payment classification purposes.

    * Schedules the ARDs for all OBRA (Omnibus BudgetReconciliation Act) assessments daily, monthly, and as needed.

    * Provides a schedule of ARDs and assessment types weeklyand as needed to the interdisciplinary team (IDT) in order to           facilitate thetimely completion of MDS sections and CAAs (Care Area Assessments) by eachdiscipline.

    * Communicates with members of the IDT as needed for timelycompletion of assessments. Expected toreport any issues           with timeliness to the Administrator.

    * Transmits assessments in accordance with currentregulations. To facilitate timelyreceipt of validation reports, will                   transmit as frequently(daily) as necessaryto obtain timely validation of MDS acceptance into the Internet Quality                     Improvement and Evaluation System (iQIES) Assessment Submission and Processing(ASAP) System data base.

    * Confirms transmission files by review and printing ofinitial and final validation reports.

    * Corrects any fatal errors immediately and retransmits theassessment until an accepted validation report is received.

    * Addresses non-fatal errors using the iQIES ASAP SystemMDS 3.0 Provider User's Guide.

    * Ensures the Business Office designee receives the initialand final validation report that acknowledges the acceptance of           the MDS intothe iQIES ASAP.

    * Facilitates the completion and updating of resident careplans with the IDT to reflect each resident's current needs.

    * Communicates with the IDT to identifyresidents in need of a Significant Change in Status assessment, based oncriteria       in the current RAI manual.

    * Participatesin all resource utilization and/or triple check meetings prior to billing ofclaims associated with payment                 assessments.

    * Assists with admission, discharge, or care of residents asneeded.

    * Attends or participates in continuing education relatedto the RAI process and related programs. Completes all assigned           training and skills competency, as determined bythe facility assessment and facility training plans.

*    Performs other administrative duties as assigned by theDirector of Nursing or Administrator, including but not limited to     QAPIactivities and participation in department or committee meetings in acooperative, effective, timely and professional     manner.

* Maintains confidentiality ofprotected health information, including verbal, written, and electroniccommunications.

* Reports noncompliance withpolicies, procedures, regulations, or breaches in confidentiality toappropriate personnel. Reports anyretaliation or discrimination to HR or compliance officer.

* Reports any allegations ofabuse, neglect, misappropriation of property, exploitation, or mistreatment ofresidents to supervisor and/or administrator. Protects residents from abuse, and cooperateswith all investigations.

* Reports any occupationalexposures to blood, body fluids, infectious materials, and/or hazardouschemicals in accordance with facility policy.

* Participatesin all life safety and emergency drills and trainings. Fulfills responsibilities as assigned duringimplementation or activation of the facility's emergency plan.

* Reports work-related injuriesand illnesses immediately to supervisor.

* Follows established infection control policies andprocedures.

* As a condition of employment,completes all assigned training and skills competency.
