Per Diem RN Supervisor - Home Care

In this role, you will have the opportunity to positively impact the lives of patients and families while supporting the success and development of our home health aides in the field.

Compensation

Pay Per Visit

  • Supervisory:$35/Post Hospital Visit: $60
  • Reassessment Visit: $75
  • Initial Admission: $90 weekday / $125 weekend

Additional Benefits

Northern Erie and Southern Niagara Counties
Schedule Expectations

This is a flexible per diem position with variable scheduling needs based on patient care demands, admissions, reassessments, and staffing coverage.

Candidates must have flexibility for:

  • Evening visits during the week
  • Weekend coverage
  • Holiday availability
  • Occasional after-hours admissions or supervisory visits

Candidates with broader availability and scheduling flexibility are strongly preferred

Responsibilities
  • Perform on-site supervisory and teaching visits to evaluate caregivers
  • Provide clinical orientation and support to caregivers on assigned home care cases
  • Participate in training and in-service education for field staff
  • Conduct initial nursing assessments and reassessments
  • Counsel patients and caregivers regarding nursing and related care needs
  • Coordinate and assign services based on patient needs
  • Collaborate with physicians to develop and revise plans of care
  • Prepare accurate clinical documentation and progress notes
  • Monitor clinical records to ensure complete and compliant documentation
  • Report changes in patient condition promptly and appropriately

  • 1 year of Home Care experience required 
  • 1 year of supervisory or management experience required
  • Valid NYS Driver's License, insurance, registration, and reliable car
  • Experience with UAS assessments
  • Familiarity with HHA Exchange (HHAx) or similar home care EMR/documentation systems
  • Managed care, MLTC, or community health experience preferred
