This position is fully remote, allowing for flexibility while maintaining a strong connection to our Community Health Clinic team. As part of a collaborative and interprofessional environment, you will work alongside other health care professionals to ensure comprehensive care is provided to our clients. • Conduct thorough psychosocial assessments to identify the emotional, mental, and social needs of clients, particularly seniors.
• Collaboratively develop and implement personalized care plans in partnership with clients and their families, ensuring that their specific goals and preferences are prioritized.
• Provide emotional support to clients and families, facilitating discussions surrounding life changes, mental health challenges, and coping strategies.
• Advocate for clients' rights and access to essential community resources, connecting them to the support and services they require.
• Engage in regular communication with interdisciplinary teams, including healthcare providers and caregivers, to ensure coordinated and holistic care.
• Monitor clients’ well-being through routine check-ins, adjusting care plans as needed, and addressing any concerns about safety and living conditions.
• Facilitate family communication and conflict resolution by meditating discussions, especially regarding care for senior clients.
• Educate clients and families about available community resources, enhancing their social engagement and overall quality of life.
• Maintain accurate and up-to-date documentation using our electronic health record (EHR) system, including assessments, care plans, and progress notes.
• Stay abreast of current social work practices, regulations, and ethical standards to ensure compliance and uphold the integrity of the profession.

• A Master’s degree in Social Work from an accredited institution is required.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• Exceptional skills in assessment, counseling, and resolving conflict.

The salary range for this position is competitive, between USD 70,000 – 100,000 per year, commensurate with experience.
