Contract: 12 weeks
Schedule: Monday–Friday, Morning Shift
Guaranteed Hours: 38 hours per week
Experience: Entry-level accepted; SNF experience preferred

About the Role

We are seeking a Physical Therapist for a 12-week contract opportunity in Sacramento, California. This role supports a skilled nursing and post-acute rehabilitation facility with an experienced rehab team and a varied caseload in a fast-paced, team-focused environment.

Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans
• Provide therapy services to support mobility, strength, balance, and functional independence
• Manage a varied caseload and prioritize patient care needs effectively
• Document therapy services accurately using PointClickCare
• Collaborate with the rehab and nursing teams to support patient recovery and outcomes

Requirements

• Active California Physical Therapist License Required
• SNF or post-acute rehabilitation experience preferred
• Strong clinical reasoning, communication, and documentation skills required
• Must be adaptable, team-oriented, and comfortable in a fast-paced environment
• Must be available for Monday–Friday morning shift schedule
