Job description

The Citadel at Riverdale Rehabilitation & Nursing Center is currently seeking LPN's for (Day Shift) including weekends to join our dynamic organization.
OVERVIEW:
Under the leadership of the Registered Nurse, the LPN will be providing coordinated care to residents in a skilled nursing facility. The objective would be to achieve patient goals and maintain high quality patient care demonstrating accountability and responsibility. Responsible for the overall operation and optimal quality of care for our residents
RESPONSIBILITIES:
  • Provides direct care responsibilities as well as supervisor responsibility for nursing assistants.
  • Administer medications and treatments timely and according to policy
  • Receive, transcribe, and carry out physician orders
  • Follows established procedure for charting
  • Patient assessments, coordination of care
  • Identifies and reports changes in condition
  • Familiarity with environmental health, safety and sanitation standards and procedures
  • Knowledge of Patient Experience and best practices, and how to best integrate with clinical care.
  • Knowledge of clinical and nonclinical acute hospital operations and continuum of care (connected care) pre/post-acute services
  • Assist in the Quality Assurance and Performance Improvement (QAPI) process

LICENSE/ CERTIFICATION
  • Valid Practical Nurse License in the State of Practice is required
  • CPR/BLS certified

EXPERIENCE:
  • Minimum 1 year nursing experience in a hospital, nursing home or other healthcare setting.
  • Strong knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care centers.
  • LTC Experience Preferred

SCHEDULE:
  • Must be able to assist under the Director of Nursing in emergencies and/ or facility coverage to meet the needs of the residents.
