Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our
Post Acute Care Unit on the night shift.
Our post-acute unit provides skilled nursing and rehabilitation services to patients recovering from surgery, illness, injury, and hospitalization, helping them regain strength and independence before returning home. As a member of our nursing team, you'll play a vital role in delivering high-quality clinical care while supporting patients through an important stage of their recovery journey.
Located in beautiful Middlebury, Vermont, Helen Porter Rehabilitation & Nursing is known for providing exceptional skilled nursing, rehabilitation, and long-term care services in a welcoming, resident-centered environment.
Schedule
- Full-Time
- 36 hours per week
- Night Shift: 6:45 PM - 7:15 AM
- Rotating weekends
- Three 12-hour shifts per week
What You'll Do
As a Night Shift RN or LPN on our Post Acute Care Unit, you will:
- Provide skilled nursing care to post-acute and short-term rehabilitation patients
- Assess, monitor, and document patient conditions and response to treatments
- Administer medications, IV therapies, and treatments as ordered
- Collaborate with physicians, nurse practitioners, therapists, and interdisciplinary team members
- Support patient recovery goals and discharge planning
- Respond to changes in patient condition and communicate effectively with providers
- Promote patient safety, comfort, and quality outcomes
- Maintain accurate electronic medical records and nursing documentation
Qualifications
Registered Nurse (RN)
- Current Vermont RN license or eligibility for Vermont licensure
- Associate's or Bachelor's degree in Nursing
Licensed Practical Nurse (LPN)
- Current Vermont LPN license or eligibility for Vermont licensure
Preferred Qualifications
- Experience in skilled nursing, rehabilitation, post-acute care, medical-surgical nursing, acute care, long-term care, or healthcare settings
- Strong assessment, communication, and critical thinking skills
- Ability to work collaboratively as part of a multidisciplinary team
- Commitment to compassionate, patient-centered care
