Job description

Licensed Practical Nurse (LPN) – Home Health
Pay: $25.00–$33.00 per hour
Job Type: Full-time, PRN, Per Diem
START DATE: ASAP


We are seeking a compassionate and skilled LPN to join our home health team. In this role, you will provide high-quality nursing care to patients receiving services through our agency. Responsibilities include patient assessments, implementing treatment plans, documenting care, administering medications, and monitoring for changes in condition. You will also support patients and families through education and ongoing communication.

Responsibilities
• Provide nursing care to ill, injured, or disabled patients in the home setting
• Perform assessments and contribute to patient treatment plans
• Monitor, document, and report changes in patient symptoms or behavior
• Administer medications and monitor for side effects
• Record vital signs and maintain accurate, up-to-date medical records
• Communicate with physicians and specialists regarding patient care
• Educate patients and families on health maintenance and disease prevention
• Facilitate referrals to other healthcare professionals or facilities
• Offer emotional support to patients and family members

Qualifications
• Previous nursing experience required
• Proficiency in:
• Tube feedings
• Wound care
• Tracheostomy care
• Ventilator management
• Ostomy care
• Familiarity with medical software and equipment
• Strong critical thinking and problem-solving skills
• Ability to build rapport with patients and families
• Flexibility and adaptability in a home-based care environment
