Job description

LaSalle Network is hiring a dedicated Care Coordinator to deliver essential services to aging and vulnerable populations across the community. This Care Coordinator position is perfect for someone who enjoys building relationships, coordinating care and ensuring clients receive the resources they need to thrive.
The Care Coordinator will assist with assessments, case management, care planning and service coordination while maintaining compliance and accurate records. This role requires strong interpersonal skills, attention to detail and the ability to balance multiple client needs.
Responsibilities:
  • Assess client needs and develop comprehensive care plans
  • Provide case management and monitor ongoing services
  • Coordinate referrals and connect clients with appropriate resources
  • Maintain up-to-date client files and documentation
  • Ensure compliance with program requirements and timelines
  • Collaborate with internal teams and community partners
  • Travel to client locations within assigned areas
  • Participate in trainings and maintain required certifications
  • Support continuous service improvement initiatives
  • Communicate effectively with clients, families and stakeholders

Requirements:
  • Bachelor's degree in a related field required
  • Strong communication and interpersonal skills
  • Ability to manage multiple clients and priorities effectively
  • Experience with data entry and case management systems
  • Critical thinking and problem-solving skills
  • Valid driver's license and proof of insurance required
  • Willingness to travel locally for client visits
  • IDOA Certification required upon hire
