Job Summary:
The P.A.T.H. (Prevention, Awareness, Trust, Health) Program Facilitator is responsible for assisting with the planning and implementation of the P.A.T.H. Program, whose focus is to expand access to comprehensive, age appropriate, and inclusive sexuality education that supports informed decision-making, strengthens youth protective factors and promotes healthy relationships and positive identity development among youth across the Commonwealth. The P.A.T.H. Facilitator provides education to youth in the priority populations using evidence-based curricula and promising practice. The P.A.T.H. facilitator will work with the program Director, and Massachusetts Department of Public Health (DPH) to evaluate outcomes and measures of success.
Duties and Responsibilities:
  • Provide Evidence-Based Programming (EBP) for the teen pregnancy prevention curriculum.
  • Maintain fidelity to EBP through outcome measuring, data collection, tracking, and analysis.
  • Recruit youth, and plan and implement educational sessions for them.
  • Represent the agency in appropriate community meetings to raise awareness of this project, educate community agencies and individuals, and collect information for the project.
  • Assist in setting annual project goals and objectives in collaboration with the funding agency.
  • Attend internal and external meetings and training sessions as required by the agency or its funders.
  • Ensure the timely collection and submission of program reports and data.
  • Ensure the timely submission of all agency paperwork in accordance with agency policies.
  • Recruit, train, and oversee project interns and volunteers, in conjunction with the PREP Program Director.
  • Promote the values of safety, cooperation, communication, and inclusiveness in all agency activities

Minimum Qualifications:
  • 2 years of experience in Public Health. human services, social work-related fields preferred
  • Experience facilitating group activities with youth is required.
  • Demonstrated evidence of ability connect with youth is preferred.
  • Experience in using Evidence Based Programming is preferred.
  • Knowledge of Springfield/Holyoke area is preferred
  • Bi-ling-bi-cultural strongly preferred
  • Commitment to community collaboration and working with the diverse communities
  • Excellent writing, computer, and people skills
  • Commitment to the goals and mission of the agency

  • Valid driver's license and reliable transportation required
