Minimum Requirements

  • Bachelor's, Master's or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Indiana Physical Therapy license required prior to start of assignment.
  • BLS certification from AHA or American Red Cross may be required.
  • Outpatient clinic experience preferred, including evaluation and plan of care development for musculoskeletal and post-operative patients.
  • Experience with documentation, goal writing, letters of medical necessity, and charge entry in outpatient EMR systems preferred.
  • Demonstrated ability to supervise and delegate to therapy assistants and students preferred.
  • Maintain professional licensure and complete state-mandated continuing education as required by Indiana Practice Act.
