Job Overview and Qualifications

This role involves providing coordinated nursing care to patients across all age groups in their homes. The position requires accountability and collaboration with an interdisciplinary team to set and meet patient goals while ensuring high-quality care. You will operate under the supervision of the Clinical Manager and in accordance with physician directives.

Key Responsibilities:

  • Evaluate home care patients to identify their physical, psychosocial, and environmental needs, as documented in clinical records, case conferences, team reports, and on-site assessments.
  • Complete OASIS assessments and visit documentation in line with agency policies, ensuring clinical notes reflect ongoing communication and coordination with physicians, team members, and patients/families/caregivers.
  • Report significant findings, issues, and changes to the Clinical Manager and physician, documenting all communications and interventions appropriately.
  • Oversee and guide home health aides and LPNs/LVNs to maintain the quality and continuity of care provided.
  • Participate in on-call rotations and respond to emergency calls as per agency guidelines.

Qualifications Required:

  • Basic Cardiac Life Support certification must be obtained within 30 days of starting employment.
  • Registered Nurse (RN) license.
  • Valid Driver's License (DL).
  • Associate or Bachelor's Degree in a relevant field.
  • A minimum of 1 year of relevant experience is required.
