Job Summary:
Home Healthcare Registered Nurse (RN) position in Modesto, California. This role involves providing in-home nursing care to adult and geriatric patients in multiple counties, ensuring holistic and individualized care delivery. The position requires developing and managing interdisciplinary care plans, fostering collaborative relationships with patients, families, and healthcare providers, and maintaining compliance with policies, standards, and regulations. The assignment begins on August 31, 2026, with a schedule of dayshift hours.
Job Details:
- Schedule: Dayshift, 8 hours per day, minimum of 40 hours per week
- Required Qualifications: Valid California RN license, Basic Life Support (BLS) certification, at least 2 years of experience in home healthcare, familiarity with EPIC charting system, previous charge nurse experience preferred, community hospital or long-term acute care experience preferred, travel experience required
- Certifications: BLS, RN license in California
- Additional Information: This role entails performing routine bedside assessments, administering medications and blood products, managing peripheral IVs, central lines, ports, and feeding tubes, wound and pressure ulcer care, patient education, care coordination, and documentation. The RN will float to various areas as needed, including Stanislaus County, San Joaquin County, and Merced County, and will be on call one day per month. Orientation hours include 40 hours prior to start.
- Experience: At least 2 years of relevant nursing experience
- Licensure: Valid California RN license, uploaded within the last 2 weeks
- References: Two references obtained within the last year
- RTO Policy: Maximum of 7 days per contract; holiday requests for New Years Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day are subject to specific start and end times.
- Compliance: All required documentation must be completed within specified timeframes prior to start date.
- Submittal: Candidate must have at least 2 years of nursing experience, with supporting documentation available for licensure and certifications.
