Patient Registration Specialist
About the Role: We are seeking a Patient Registration Specialist who is passionate about delivering an exceptional patient experience and thrives in a dynamic, non-traditional telemedicine environment. In this crucial role, you will be the welcoming voice of Form Health for our new patients, acting as the primary driver of the initial enrollment journey. You will guide patients seamlessly through the earliest stages of their care, providing high-touch support to help them complete initial enrollment tasks and navigate the onboarding process. You'll work directly with patients over the phone to ensure they are set up for success, excelling at managing a high volume of calls and tasks independently to ensure a frictionless start to their weight loss journey with Form Health. This is a full-time, remote opportunity where your ability to be metrics-driven will directly contribute to our efficiency and growth.
What You Will Do:
Inbound Engagement & Enrollment Support: Act as the primary point of contact for new patients via phone and email. You will provide clear guidance and elevated support as they complete all required initial enrollment steps, explaining program requirements and the value of Form Health.Onboarding & Progress Management: Proactively support patients in completing their initial enrollment tasks within the Core platform. You will track patient progress to reduce "drop-off" during the sign-up phase, ensuring those who start via the app move forward to the eligibility review phase.High-Volume Registration & Setup by Phone: Efficiently register and onboard a high volume of new patients primarily by phone. This involves collecting essential information such as insurance details and ID cards, answering questions about the program and requirements, verifying that patients meet initial enrollment requirements, and setting up patient charts in our electronic medical record system (EMR) to ensure they are ready for the next phase of care.Insurance Verification: Verify patient insurance information to confirm active coverage and eligibility for Form Health programs.Engagement & Proactive Outreach: Perform consistent outreach to encourage new patients to complete outstanding onboarding steps.Operational Improvement: Offer valuable feedback and communicate patient experiences to internal teams (e.g., Product, Engineering, Marketing) to help improve our operational tools.
You'll Be Successful in This Role Because You Have:
  • At least two years of inbound phone experience in a fast-paced healthcare setting, patient enrollment, or high-volume customer service role.
  • A proven ability to manage a high volume of tasks independently while maintaining a focus on efficient workflows and enrollment targets.
  • Excellent verbal and written communication skills, with the ability to effectively convey information clearly and empathetically to diverse patient populations.
  • The ability to remain calm and professional while working through challenging calls or technical hurdles with patients.
  • Proficient computer skills, with the ability to quickly adapt to evolving technologies and proficiency in various platforms (e.g., Slack, Google Workspace, EMR systems like Elation, or custom-built enrollment platforms).
  • An empathetic and patient approach when working with individuals from diverse backgrounds and varying levels of technical comfort.
  • A proactive mindset, demonstrating adaptability and creative problem-solving abilities when faced with challenges.
