To provide nursing care to the residents as prescribed by the physician and nurse practitioner and in accordance with established facility protocol, local, state and federal regulations. To lead and/or direct Nurse Aides in the delivery of direct resident care.

Requirements:

DUTIES AND RESPONSIBILITIES:

1. Assess the overall status of the residents and document in the medical record. 2. Monitor the residents’ activity and provide nursing care in accordance with physician orders, care plans, local, state and federal regulations as well as established facility protocol. 3. Coordinate, prepare for and assist with new admissions, transfers and discharges. 4. Create care plan in Eldermark for each new admission. Follow up regularly to adjust care plans as needed. 5. Communicate with residents, family members, other disciplines and health care professionals regarding the status of the resident. 6. Take and record vital signs. 7. Administer medications and treatments and document accordingly. 8. Schedule job assignments and duties for Resident Aides. Make sure they are documenting resident care in Eldermark as needed. 9. Supervise weekend nursing staff. Work with Executive Director on scheduling to ensure proper weekend coverage. 10. Train all new nurses. Participate in training new Resident Aides regarding any specific clinical protocols. 11. Be observant for changes in a resident’s condition and notify physician and family, as needed. 12. Visit all residents at various times during your shift. 13. Supervise, and assist with the feeding of residents. DUTIES AND RESPONSIBILITIES: 1. Carry out all physician orders, including making out lab slips, x-ray requests and M.A.R. 16. Provide routine nursing documentation as per established facility protocol. 17. Prepare residents for going on pass. 18. Oversee and manage the “Healthy You” program with quarterly assessments. 19. Complete Incident/Accident Reports on all residents, employees and visitors as the need arises. 20. Leave the medication room and medication cart clean and well stocked for the oncoming shift. 21. Manage controlled drugs according to the process set forth by the State Board of Pharmacy. Maintain security of all controlled drugs. Manage process for destruction of drugs. 22. Report occupational exposure in accordance with established facility protocol. 23. Be knowledgeable about and promote resident rights. 24. Prevents abuse, neglect and exploitation in the elderly. 25. Promote a safe, clean environment in which the residents may live. 26. Become a participating facility team member with the residents as first priority. 27. Provide afterhours “on-call” services M-F. When unavailable, schedule PT nurses for on call. Communicate on-call schedule to ED and staff.
