Job Purpose:
- The Home Care MSW Social Worker at EHM Senior Solutions is responsible for providing comprehensive social work services to clients in their homes. This role involves assessing the psychosocial needs of clients, developing care plans, and coordinating with healthcare providers to ensure the delivery of quality care. The social worker will support clients and their families in navigating healthcare systems, accessing community resources, and improving their quality of life.
Key Responsibilities:
- Conduct thorough assessments of clients' psychosocial needs and develop individualized care plans.
- Provide counseling and support to clients and their families to address emotional, social, and financial challenges.
- Collaborate with healthcare providers, including nurses and physicians, to coordinate care and ensure a holistic approach to client health and well-being.
- Assist clients in accessing community resources and services, such as financial assistance, transportation, and support groups.
- Advocate for clients' rights and needs within the healthcare system and community.
