Schedule:ย Monday through Friday 8:00 AM to 4:00 PM/ 9:00 AM to 5:00 PM

Whatย youโ€™llย do:ย 

  • Responsible forย Informing and coordinating community resources including coordination among primary, preventative, and chronic care providers.ย 

  • Assistย inย locatingย and accessing non-Medicaid community supports and resources that are related to achieving Individual Support Plans (ISP) goals.ย 

  • Assistย inย locatingย options for renting or purchasing a personal residence,ย assistingย withย purchasingย furnishings for the personal residence.ย 

  • Provide instruction and counseling which guides the participant in problem solving and decisionย making.ย 

  • Advocate on behalf of theย participant.ย 

  • Support the person in preparing,ย participatingย inย and implementing the Individual Support Planย (ISP).ย 

  • Provide training on the Individual and Family Directed Supports Option, if the participant isย considering directing services andย supports.ย 

  • Assistย management with guidanceย ofย the individual budget.ย 

  • Assistย with coordinating of services with the Financial Supports Services provider, if the participantย is self-directing services under the Employer of Record Model, including guidance on use of theย individual and family directed budget (self-directed budget).ย 

  • Provide information on recruiting, hiring, managing, training, evaluating, and changing supportย staff, if the participant is self-directing services.ย 

  • Assistย with the development of schedules and outlining staffย duties, ifย the participant is self-directingย services.ย 

  • Assistย with understanding staff qualifications, documentation and record keeping requirements,ย if the participant is self-directing services.ย 

  • Provide on-going information to assure that participants and their families/representativesย understand the responsibilities involved with self-direction, including reporting on expendituresย and other relevant information and training.ย 

  • Coordinate services with the Agency with Choice if the participant is directing services underย the Agency with Choice Model.ย 

  • Be available to work flexible hours including nights, weekends and overtime asย required.ย 

  • Depending on the person you support, you mayย be requiredย to:ย 

  • Administer medications. Training is provided.ย Nursingย team supports you.ย 

  • Provide transportation to and from designated sites.ย 

  • Assistย people with mobility needs,ย e.g.ย utilizeย transfer techniques,ย aย Hoyerย liftย and/or gait belt.ย Training is provided.ย 

What you need:ย ย 

  • Must be at least 18 yearsย of ageย ย 

  • Possess a valid driverโ€™s license for a minimum of two consecutive yearsย 

  • Pass criminal recordย andย healthcare registry checkย ย 

  • Pass driving record check if providing transportationย ย 

  • High school graduate or equivalent (GED)ย ย 

  • Commitment to achieve competency in required knowledge, skills, and abilitiesย ย 

  • Note:ย Currentย First Aid/CPRย blendedย or classroomย certificationย isย required.ย If you do not currently have certification from an approvedย provider, our American Red Cross certified instructors will provide you the training.ย 

  • Ability toย lift upย to 40 pounds independently (per policy)ย
