Job description

Summary

The Home Health Aide (HHA) at Heritage Home Healthcare provides compassionate, high-quality in-home personal care and support services to clients in the Albuquerque, NM area under the supervision of a Field Supervisor and Registered Nurse. In this role, the HHA assists with Activities of Daily Living (ADLs) and promotes client safety, comfort, independence, and dignity in the home setting while maintaining compliance with NM DOH and CMS regulations.

Essential Functions
  • Provide personal care services including bathing, grooming, oral hygiene, dressing, toileting, and incontinence care
  • Assist with mobility, transfers, ambulation, and positioning using proper body mechanics
  • Assist with feeding and meal preparation according to dietary guidelines and care plans
  • Provide skin care and support pressure injury prevention
  • Maintain a clean, safe, and organized environment related to client care
  • Support clients with cognitive impairments, dementia, or behavioral health needs per the Plan of Care
  • Observe, document, and report changes in client condition, behavior, or environment to the supervising nurse or agency staff
  • Follow the Plan of Care and communicate when care needs fall outside scope or instructions
  • Follow infection control standards and universal precautions
  • Use personal protective equipment (PPE) appropriately
  • Maintain HIPAA compliance, patient rights, and professional boundaries
  • Report suspected abuse, neglect, or exploitation per NM law and agency policy
  • Participate in required training, competency evaluations, and in-service education
Job Qualifications
  • High school diploma or equivalent
  • Completion of a state-approved Home Health Aide training program or CMS-equivalent competency
  • Ability to pass background check, abuse registry check, and drug screening
  • Valid driver's license and reliable transportation
  • Current CPR/First Aid certification or ability to obtain
