Job description

Case Manager RN | Tallahassee, FL

Schedule:

  • Evening Shift: 12:00 PM – 8:30 PM EST
  • 1 weekend per 4-week schedule

Shift Differentials:

  • Evening: +$2.50/hr
  • Weekend: +$2.00/hr
  • Combined: +$4.50/hr


Sign-On Bonus: $10,000

Relocation: Available


Key Responsibilities:

  • Conduct comprehensive psychosocial and medical assessments
  • Develop, coordinate, and document individualized case management plans
  • Lead interdisciplinary care coordination and team meetings
  • Evaluate admissions for medical necessity and level of care
  • Perform utilization reviews and communicate with payors


Experience required:

  • Minimum 3 years of recent Case Manager experience (within last year) in acute care
    OR
  • 3 year's experience in Med/Surg, Tele, Neuro, ICU, PCU, or ED
  • Home health/insurance candidates considered with:
    • 3 years total acute care experience
    • At least 1-year acute care within the last 5 years


Education:

  • Associate Degree or Diploma in Nursing (Required)
  • Bachelor’s Degree in Nursing (Preferred)


Licensure & Certifications:

  • Active Florida RN license or Compact License (FL license required within 90 days if Compact held)
  • APRN license accepted if current
  • Certification in Case Management, Nursing, or Utilization Review (Preferred)
