Job description

Job Summary * (In three or four sentences summarize the job's purpose and primary focus.)
Responsible for repatriation of members from non-core hospitals to KP core hospitals, and ensuring our members are in the right place to receive the right care, at the right time by our PMG and Health Plan teams. Utilizes expert medical and systems knowledge to assist providers in navigating our complex care delivery system to improve the members care experience. Provides length of stay management for members in non-core facilities in collaboration with QRM's non- core team, using Care Without Delay (CWD) principles. These functions combine to ensure our members receive the highest quality care and our physicians receive the best decision support in the most efficient and effective manner.
Major Responsibilities/Essential Functions * (In order of importance, list 5-7 primary job duties this position is responsible for achieving. Each statement should relate to an end result or objective and not a task list.)
1.Accountable for the performing the duties of the ECM hub:
a.Repatriation of medically stable patients from non-core Emergency Departments (EDs) and Inpatient/Observation status to KP core hospitals with both the ECM Hub team and our QRM partners.
b.. Works collaboratively with ECM non-core physicians and QRM's non-core team to provide both length of stay management and repatriations KP core facilities utilizing CWD principles.
c.Coordination of care including timely venue management for patients at home, Medical Office Buildings, Acute Care Centers (ACC), Clinical Decision Unit s(CDU), Skilled Nursing Facilities and home who require further acute care services in a higher acuity venue.
d.Consultation for PMG providers (Primary and Specialty Care) to assist with developing ideal care plans.
e.Physician medical advice and triage recommendations to the advice nurses to determine the best place of care for high acuity medical problems.
f.Supports ECM hub staff with implementation of Hub functions.
2.Review same day and future requests for admissions to ensure appropriateness based on in-patient criteria and timing of admission using CWD principles.
3.Maintain an in depth understanding of our care delivery system and our contractual arrangements to provide the most effective care coordination and support.
4.Maintain an accurate understanding of all pertinent compliance and regulatory issues related to patient transfer.
5.Remain abreast of in-patient admission criteria and levels of care.
6.Collaborate with Care Without Delay (CWD) leaders to ensure the most appropriate resources are utilized for our patients.
