Your Impact
  • Partner with care team members to develop and implement patient-centered clinical plans.
  • Deliver wrap-around services through home visits and collaborative care alongside providers and RNs.
  • Perform key clinical tasks in the field- chronic condition support, wound care, phlebotomy, medication review, and more.
  • Engage hard-to-reach patients, bringing them vital care and support directly at home.

Requirements
  • Licensed Practice Nurse, unencumbered license certification in the state of employment
  • Ability to conduct independent home visits
  • Experience providing care for the whole person, including caring for adults and seniors with high medical and social complexity
  • Current CPR and BLS certifications required
  • Intermediate to advanced computer and software skills, including Patient Health Exchange Platforms, Electronic Health Records, and Outlook
  • Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required.
  • DRIVER REQUIREMENTS: Licensed for a minimum of 5 years. No vehicle-related suspensions, DUI, reckless driving, leaving the scene of an accident, vehicular manslaughter, felony convictions, or 3 or more moving violations within the last 5 years. No more than 2 moving violations or 1 at-fault accident in the last 3 years.

ConcertoCare requires all "frontline workers" to be fully vaccinated and to provide records for validation. Medical or religious exemptions will be considered contingent upon the review of appropriate documentation.
Base Salary/ Wage Range $35.00 to $42.00/hour with annual bonus. Compensation for the role is commensurate with the candidate's qualifications, skills, competencies, and experience and may fall outside of the range shown.
