Job description

The Healthcare Coordinator is a vital member of the residential support team, responsible for overseeing the health and wellness needs of residents recovering from substance use disorders.  The position ensures that residents receive appropriate medical attention, preventative care, and support in navigating the healthcare system.  The coordinator monitors communicable disease and other health risks, assists with basic health assessments and works closely with staff, external providers and community resources to coordinate care.  
JOB QUALIFICATIONS/RESPONSIBILIITES/SKILLS
The ideal candidate is experienced in community based or residential settings, knowledgeable about substance use disorder and comfortable working with individuals with medical and behavioral health needs.  The role requires strong organizational skills, health knowledge and the ability to advocate for resident care in a respectful, culturally sensitive manner.  
KEY RESPONSIBILITIES
•    Monitor residents for signs of communicable disease, physical health issues and urgent health needs
•    Conduct or assist with basic health screenings and wellness assessments upon intake and regularly thereafter. 
•    Coordinate medical appointments and referrals to external providers
•    Maintain accurate confidential health documentation in record (EHR) systems
•    Educate residents and staff on general health topics including communicable disease prevention, hygiene and medication adherence
•    Work preformed primarily on-site at the residence with occasional offsite coordination
QUALIFICATIONS
•    Minimum two years’ experience in providing Health education in SUD or related field. 
•    Strong knowledge base in the provision of education, risk reduction, counseling and referral services regarding HIV tuberculosis, viral hepatitis, sexually transmitted infections and other transmissible infections.
•    Valid NYS Driver’s license
•    Knowledge of basic keyboarding, Windows and Microsoft office  
•    Excellent written and oral communication skills
•    Must be able to work independently
PHYSICAL DEMANDS/WORK ENVIRONMENT
Work conditions may include exposure to inclement weather and noise.  Position may require exposure to electronic equipment including computers, printers, fax and copy machines and cleaning equipment.  Position requires use of a motor vehicle, driving on city streets and/or highways, exposure to inclement weather and traffic.  Employees are at take all precautions and follow all laws while in these environments.  Requires walking, climbing stairs and periods of standing.  Must be able to lift up to 50 lbs.  Must be comfortable working in high paced or emotionally demanding situations.
