Job description

Position Summary:
Transport clients to and from PHP/Outpatient program on a daily basis. Provides client transportation, maintains log of patient transportation, mileage and current patient information sheets. Position will also provide direct patient care, assisting with monitoring client interaction, assisting with groups & cafeteria/lunches, etc. Organizes workload in an efficient manner. Performs patient care duties with attention to detail, accuracy, safety and client comfort.
Position Responsibilities:
Clinical / Technical Skills (40% of performance review)
  • Ability to operate a moving vehicle, while obeying local and state driving laws
  • Ability to transport patients from the pick-up/drop-off locations to the hospital and back, while adhering to a strict schedule
  • Perform regularly scheduled inspection checks on transportation vehicle before leaving hospital grounds.
  • Transport vehicle to off-site locations for routine maintenance on company vehicle.
  • Manage patient interactions during transportation between pick-up/drop-off locations and hospital
  • Demonstrates good judgement during transportation of patients in situations of crisis.
  • Communicates with supervisor to report incidents of concern upon return to hospital.
  • Documents incidents of concern while interacting with patients following return to hospital.
  • Maintains the cleanliness of the company vehicle following each use.
  • Must report all moving violations before the end of the worked shift.
  • Ability to present a positive image of the organization to members of the community.
  • Maintain the maintenance logs for the van, and report any vehicle problems to department supervisor in a timely manner.
  • Maintain cleanliness of soda and coffee machine in the Outpatient area.
  • Manage meal transportation to the Outpatient area.
  • Maintain Reception and locker logs (patient sign in sheets)
  • Conduct contraband searches and wand upon arrival to Outpatient area.
  • Assists in outpatient department while not performing transportation duties.
  • Perform other duties as assigned.
