Job Summary: We are seeking a highly skilled and compassionate Telehealth RN Case Manager to join our dynamic healthcare team. The ideal candidate will be responsible for coordinating and managing patient care remotely, utilizing telehealth platforms to provide efficient and effective healthcare services.   • Educate patients and caregivers on health management strategies, medications, and self-care techniques.
  • Document all patient interactions, assessments, and care plans accurately and timely within electronic health records (EHR) systems.
  • Coordinate referrals to community resources and support services as necessary.
  • Stay informed about current healthcare trends, telehealth regulations, and best practices in case management.
Requirements:
  • Current unrestricted RN license in Michigan.
  • BLS Certification required
  • Minimum 2 years of clinical nursing experience, with a background in case management preferred.
  • Certification in Case Management (CCM) preferred.
  • Proficiency in using telehealth platforms and electronic health records (EHR) systems.
  • Excellent communication skills, both verbal and written.
