Pilsen Medical Clinic & Associates is actively seeking Independent Contractor (APNP) Advanced Practice Nurse Practitioner to complete home visits around the Chicagoland area.   • Assure compliance with licensing, certification and accrediting bodies.
  • Represent company and the clinic as reasonably required at internal and external meetings or events.
  • Maintain working equipment and supplies.
  • Coordinate patient care with patient's associated home health agency.
  • Review patient case document in an timely manner.
