Are you a compassionate and dedicated Social Worker with healthcare experience? Join our mission-driven team at Burlington Health and Rehab and make a lasting impact on the lives of our residents. We're seeking a a Strong candidate to fill a Social Services Director position- Social Worker with experience in long-term care preferred to support the social and emotional well-being of our residents.

Key Responsibilities, but not limited to: 
  • Support residents in maintaining or improving physical, emotional, and psychosocial well-being

  • Identify and assess individual social and emotional needs

  • Create and update personalized care plans

  • Participate in admission planning, discharge coordination, and transfers

  • Collaborate in interdisciplinary care plan meetings

  • Partner with finance to ensure residents understand documentation and financial needs

  • Coordinate discharge planning, including DME and home care services for safe community reintegration

Qualifications:
  • §483.70(o)(1) An individual with a minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology; and
  • §483.70(o)(2) One year of supervised social work experience in a health care setting working directly with individuals
  • Experience working with aging and diverse populations
